‘Fy Iechyd, Fy Newis’ – cymorth gyda gofal iechyd yn eich cymuned
Mae Bwrdd Iechyd Prifysgol Hywel Dda wedi creu adnoddau, gan gynnwys fideos o’r enw “Fy Iechyd, Fy Newis”, i roi gwybodaeth hawdd i’w deall i bobl am gael mynediad at Ofal Sylfaenol yn eu cymunedau.
Mae gwasanaethau Gofal Sylfaenol yn cynnwys practisau meddygon teulu, Fferyllfeydd Cymunedol, gwasanaethau deintyddol y GIG a Phractisau Optometrig (cyfeirir atynt yn aml fel Optegwyr). Mae’r rhain i gyd yn rhan hanfodol o’r ffordd y darperir gwasanaethau i’n poblogaeth. Gall gwybod sut a phryd i ddefnyddior gwasanaethau hyn sicrhau bod pobl yn gwneud y dewis cywir ynghylch pa
wasanaeth y gall fod ei angen arnynt a chael cyngor neu driniaeth briodol yn gyflymach.
“Mae’n wirioneddol bwysig i gleifion ddeall pa wasanaethau sydd ar gael yn lleol a sut a phryd i gael mynediad atynt, ac wrth gwrs i feddwl am yr hyn y gall cleifion ei wneud eu hunain i gynnal eu hiechyd a’u lles eu hunain”, meddai Jill Paterson, Cyfarwyddwr Gofal Sylfaenol, Cymunedol a Gofal Hirdymor ar gyfer Bwrdd Iechyd Prifysgol Hywel Dda. “Rydym wedi creu’r adnoddau ‘Fy Iechyd, Fy Newis’ gyda’r nod o rymuso pobl i fod yn gyfrifol am eu hiechyd. Bydd gwybod pryd a ble i geisio cymorth yn galluogi pobl i gael y gofal cywir ar yr amser cywir. Gobeithiwn y bydd yr adnoddau hyn yn helpu unigolion a’n cymunedau lleol i wneud y defnydd gorau o’u gwasanaethau lleol fel y gallant fod yn fwy effeithlon a hygyrch i bawb”.

‘My Health My Choice’ – help with health care in your community
Hywel Dda University Health Board has created resources, including videos called “My Health, My Choice”, to provide people with easy-to-understand information about accessing Primary Care in their communities.
Primary Care services include GP practices, Community Pharmacies, NHS dental services and Optometric Practices (commonly referred to as Opticians). These are all an essential part of the way in which services are provided for our population.
Knowing how and when to use these services can ensure that people make the right choice about which service, they may need to access and get appropriate advice or treatment more quickly.
“It is really important for patients to understand which services are available locally and how and when to access them, and of course to think about what patients can do themselves to maintain their own health and well-being”, said Jill Paterson, Director of Primary Care, Community and Long-Term Care for Hywel Dda University Health Board. “We have created the ‘My Health My Choice’ resources with the aim of empowering people to take charge of their health. Knowing when and where to seek
help will allow people to get the right care at the right time. We hope that these resources will help individuals and our local Communities to make best use of their local services so that they can be more efficient and accessible for everyone”.
The “My Health My Choice” resources and short bilingual videos are available to watch online at https://hduhb.nhs.wales/primary-care-information. Posters with QR codes linking to these resources will be available at GP Practices, Community Pharmacies, NHS Dental Practices, Optometrists, Hospitals, and Community Centres. Information is also being sent to schools and will be used on the Health Board’s social media accounts on Facebook, Instagram and X.
