New patient registration

In order to register with the practice you should bring two forms of I.D, and know your NHS number. The receptionist will ask you to fill in a medical questionnaire and will offer you a registration medical appointment, in which you will have to provide a urine sample.

We would like you to attend this appointment as it allows us to record certain data including your medical history, allergies and family history. Having this information will make medical staff better aware of your needs.

If you prefer a particular type of doctor e.g. female for a female patient and vice versa we will note this and do our best to respect your choice. However not all the doctors in the practice provide all services and specific doctors may not be immediately available.

To register with our surgery you need to live within the practice area and not be currently registered in another surgery in the Llanelli area.

You and Your Doctor

Patients are registered with the practice, not an individual GP. You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. We will do our best to respect your choice, however, not all the doctors in the practice provide all services and specific doctors may not be immediately available.

Change of personal details

Change of personal details forms are accessible on our website, below.

It is important to keep us informed of any changes to your personal information, such as your name, address, telephone number, and mobile number. This allows us to easily contact you or refer you to a hospital consultant if needed.

If you move outside of our practice area, you will need to register with a doctor in the new area. Please be aware that we do not accept patients who reside outside of our practice boundary.

You can obtain change of personal detail forms from the reception desks at both of our surgeries.

Updating your contact details

If you change your name, address or telephone number, please let our receptionists know when attending surgery or in writing. This will ensure your records are kept up to date enabling us to contact you if and when required. If you move outside the practice area you may need to find a doctor in your new area.

Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document.

If you are changing address, please provide a copy of a document as proof of address.
