ADHD & Autism Screening
Please read the information carefully before submitting your screening form to the surgery.
Who is this form for?
This form is for any adult (AGED 18 OR OVER) who would like to be referred to the Hywel Da Adult ADHD (attention deficit hyperactivity disorder) or Adult Autism service.

Who is unable to access this screening service?
1) We cannot accept referrals for children (<18 years old) via this screening form
2) We are unable to refer children for ADHD assessment, from the surgery. ADHD referrals in children are done via the health visitor/nursery/school/college.
3) If you believe your child has autism or any other neurodevelopmental concerns, please contact the surgery directly for an appointment.
4) If you have got concerns about your mental health and require input for this, we would advise you contact the surgery directly to make an appointment to discuss this and not via this screening form as they will not be responded to on an individual basis.
What services are on offer?
Autism – pre-diagnostic counselling, diagnostic assessment, and signposting. MDT approach with speech and language therapy, occupational therapy, and psychology. Waiting time (from latest figure May 2023) – 1.5 – 2 years.
ADHD – new diagnosis of ADHD (or those formerly with diagnosis who are not on medication), support and medication as required. ADHD service will also recognise private diagnoses of ADHD with relevant paperwork submitted. Waiting time (from latest figures August 2023) – 3.5 to 4 years (for
new diagnoses)
Please note – at present we are NOT as a surgery accepting into shared care provisions with private healthcare providers.
We will facilitate the referral into the ADHD service using private diagnosis with the relevant paperwork and the ADHD service will be responsible for medication provision, monitoring and titration of medications.

If you are experiencing significant issues with your mental health and need to access MH resources please contact the surgery any day, at 08:30 by telephone (01554 774401).
Alternatively call 111 and press option 2 for a mental health practitioner/ attend Glangwili Hospital or Prince Philip Hospital (minor injury unit) and ask for the crisis team.
All forms will be screened by our doctors and forwarded onto the correct referral service. We will contact you to let you know that your screening form has been received.
